New Patient Registration

We are currently accepting new patients on our list. You can start your initial registration in less than 5 minutes. We need your name, surname, DOB and contact details to get you in our system. See more information on how.

If you live in our practice catchment area (please see our catchment area map further down this page) and wish to register with us, please complete all of the forms in our registration pack.

Download, print complete and return all the appropriate forms to the surgery, depending on the age of each patient wanting to register. You can either drop them in the surgery with a copy of the following documentation;

  • One photographic proof of identification, either a photo driving licence or passport
  • And one proof of your current address, which can either be a recent utility bill (no less than 2 months old), or a mortgage / bank statement or a letter from your landlord or a rental agreement.

All our new patients are offered a health check with a member of our clinical team to check that all required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Important information

  • Registration will not be complete until you have completed all necessary forms attached and attended the surgery with proof of identity.
  • Where there are several people from one household registering you must provide each person's individual registration application with supporting evidence separately. 
  • You must allow at least 10 working days for your account to be registered. Please ensure that you have completed all the relevant forms included in the registration pack before submission. Incomplete applications will delay your registration.

Blood & Organ Donor registration

When filling out the GMS-1 registration form to join us as a new patient, you will be offered the chance to sign up to the NHS national blood door and organ donor schemes.

Temporary patient registration

If you are ill while away from home, or if you are not registered with a doctor but need to see one, you can receive emergency treatment from a local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the practice. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
